Under this program, case managers put in place individualized care plans for adults living in Greater Toronto; the care plan is based on a formal standardized client assessment. Case managers help clients navigate the health and community support services system effectively so as to direct them to the appropriate services. Case managers help clients with the application process for social, financial, legal and immigration services. They may also provide counselling and escort clients to appointments. The ultimate goal of the program is to improve clients’ quality of life and maximize their independence.
