Case Management

CAH case managers help clients navigate the healthcare system and community support services effectively. They guide individuals and/or caregivers toward the most appropriate resources based on their needs, whether these are CAH services or those offered by community partner organizations.

We all work together to support the Francophone community in navigating the continuum of health and social services in French.

Who can benefit from case management?

CAH service clients
Each client receives a personalized care plan based on a standardized assessment of their level of autonomy and needs. In some cases, the case manager may also accompany the client to medical appointments.
Francophone adults in the community
Case managers also support any Francophone adult in navigating the healthcare and community support system to access appropriate services.
