Transitional Care Unit (TCU)

For several years, CAH has worked closely with Ontario Health and the Ontario Ministry of Health. This collaboration aims to provide a continuum of French-language care, accessible at the right time and in the most appropriate setting based on seniors’ needs.

The Transitional Care Unit responds to a well-known reality: some hospitalized seniors no longer require acute care, but cannot return home immediately due to a lack of support services or home modifications needed for their safety and independence. This unit therefore provides an essential step in a smooth and effective care pathway between hospital discharge and returning home.

Since September 2018, the Francophone community of the Greater Toronto Area has benefited from two private rooms at Place Saint-Laurent, within a respite and transitional care unit exclusively for Francophones. Hospitals can refer Francophone patients to ensure a safe, human, and fully French-language transition.

Free of charge.

Funding

The Transitional Care Unit is fully funded by Ontario Health. This initiative is designed to facilitate transitions for Francophone seniors after hospital discharge.
