Home Support

The greatest wish of older adults is to remain at home for as long as possible.

Support in French

CAH’s Home Support program (AAD) provides French-language assistance to seniors in Toronto who experience difficulty completing certain daily tasks or who are living in isolation.

Through a personalized assessment, our care attendants tailor their support to each client’s specific needs while preserving their autonomy and safety.

Rate

$12/hour

Subsidies available upon proof of income

Hours

Monday to Friday, 9:00 a.m. – 5:00 p.m.

Funding

A subsidy may be granted upon submission of income documentation. Approval depends on the availability of CAH funding.

Needs assessment and eligibility

A CAH case manager conducts a standardized assessment.
They also evaluate the client’s physical condition and home safety environment.
The results determine the recommended number of Home Support hours.
Needs are reassessed at least once a year, or whenever there is a significant change in condition.

For a simpler, more independent daily life

Nature of services

CAH’s Home Support services promote physical and cognitive activation and support healthy aging. They encourage clients to remain actively engaged in their daily routines. During visits, and based on individual needs, our care attendants assist with simple yet essential tasks to ensure a safe and healthy living environment.

Examples of services

Light housekeeping

Care attendants assess light household tasks that can be completed with the client’s participation, suggesting safe methods for performing them. When needed, staff may also assist with grocery shopping and light meal preparation.

Physical and cognitive activation

Physical exercise and cognitive stimulation sessions validated by Glendon College, York University.

Safety calls

Upon request, clients may receive regular safety check-in calls to support home safety and well-being.
