Heritage Support Services (SSH)

Heritage Support Services (SSH) provide daily assistance to seniors aged 55 and over living at Place Saint-Laurent. This program supports residents with reduced physical or cognitive autonomy to remain safely in their own apartments.

Rate

Free of charge.


Only meals are paid: $8 per meal.

Funding

The SSH program is funded by Ontario Health (Ministry of Health).

Eligibility

Eligibility is determined through a standardized assessment conducted by a CAH care coordinator.

Assessment of needs and eligibility for the personal care assistance program – Heritage Support Services (SSH)

During an initial visit in the resident’s apartment:

The CAH care coordinator completes a standardized assessment
A physical and environmental safety evaluation is conducted
The results determine the number of SSH service hours required
Needs are reassessed annually or whenever there is a significant change

The results determine the number of SSH service hours required
Needs are reassessed annually or whenever there is a significant change

Nature of SSH services

Eligible residents may receive:

soins personnels

Personal care

Assistance with bathing, hygiene, and dressing.

Physical and cognitive activation

Structured activities supporting mobility and mental stimulation.

Medication support

Assistance with medication intake.

Nutrition

Help with breakfast preparation and access to low-cost hot meals for lunch and dinner in the dining room.

Additional services included in SSH

As part of the SSH program, eligible clients may access additional services based on their needs, such as Home Care (AAD), accompaniment services, transportation, and the Day Program.

The Day Program provides respite for families: French-language activities, professional supervision, and a hot meal for your loved ones — in both Toronto and Oshawa.

Personalized in-home support to help seniors remain active, independent, and safe.

Individual health information and education to support informed decision-making

Regular safety calls or visits, depending on risk level

Care coordination support to navigate the healthcare system

Foot care (for eligible clients) in partnership with the Francophone Centre of Toronto

24/7 support worker on site for emergencies
